How to deliver effective integrated community services for the frail elderly Perspective and Reflections from the UK
Section 1: The concept In both China and the UK there is a clinical and operational need to shift the focus from treatment to longer-term holistic needs of the frail elderly people.Frailty rather than being a series of conditions is in effect a long-term condition and the most significant multi-morbidity challenge across the world today.This chapter considers the evidence-based practice from the UK and considers how this can be applied operationally in China as well as highlighting where there are similarities between the 2 countries and the approach being taken in China and the UK and the relevance of such an approach internationally.
