Subthalamic nucleus deep brain stimulation for I Parkinsons disease:the relationship between locations of oscillatory activity and optimal site of stimulation

  Objective: to explore localization of oscillatory activity associated with the optimal contacts of subthalamic nucleus deep brain stimulation(STN DBS)which results in the best motor benefits.Methods: Twenty-five parkinsonian patients who underwent electrode implantation into the STN for motor symptoms were investigated.Microelectrode recordings were performed in the STN.Neuronal oscillatory activity was evaluated using power spectrum analysis.Coherence analysis was used to study the relationship between neuronal oscillatory activity and EMG.Clinical outcomes were evaluated.The position of optimal electrode contacts was assessed and was compared to the location of oscillatory activity.Results: Of the total 208 neurons,57 neurons showed significant oscillatory activity and 53 were localized in the dorsal portion of the STN.There were oscillatory neuronal activities coherent with either limb tremor or rigidity.There was no significant difference between the position of 40 optimal contacts and the dorsal margin of the STN.Conclusions: The abnormal oscillatory activities play an important role in the pathophysiology of PD.The positions of the best contacts correlate with the locations of the oscillatory neurons supporting the prediction that stimulation of the dorsolateral oscillatory region leads to an effective clinical outcome for STN DBS.
