Telehealth Enhancement of Hospital Outreach for the Aged and Chronic Disease Management:A UTAUT-based Model for Review of Patients Acceptance

　　Chronic disease has been on the decline for the past few decades.However, with the increasing percentage of older people its presence is being felt.Technology is expected to assist in meeting increasing demands for chronic disease support but adoption of Telehealth and Telecare has to date been disappointing.In this paper a model for further research on adoption of home Telehealth for Heart Failure (HF) derived from UTAUT is presented.In the model two (2) additional constructs have been added to the UTAUT set of constructs.
