INFORMATION OVERVIEW IN THE CARE CHAIN:VISUALISATION SUPPORT IN HIGHLY SPECIALISED HEALTH CARE
In the Stockholm region, the highly specialised health care within the upper gastrointestinal tract was in 2004 concentrated to Gastro, a gastroenterology department at Karolinska University Hospital. To provide the best possible care for the patients, to reduce travel for the patients and involved medical personnel, and to share resources and disseminate knowledge between different hospitals within the county council, Gastro developed and implemented a care chain, including multi-disciplinary video-mediated meetings between hospitals. In collaboration with physicians at Gastro, and based on long term field studies, we have developed a shared workspace, the Clinical Journal, that supports information availability during different activities in the Care Chain. We describe the Care Chain, the Clinical Journal, and how the Clinical Journal can be used in different parts of the Care Chain.
