Measurement of Entrance Skin Dose for Diagnostic X-ray Radiographic Examinations and Establishment of Local Diagnostic Reference Levels
The radiation dose resulting from medical diagnostic procedures is the largest contributor of the population dose because a large number of x-ray examinations are conducted every year globally.The patient dose resulting from an x-ray diagnostic procedure depends on a number of parameters such as energy of the x-ray beam,beam current,exposure duration,type of image recording system,technique of examination and the type of x-ray generator.Although national and international dose reference levels are available,measuring and recording the patient dose data under the realistic conditions of exposures and establishing local Diagnostic Reference Levels (DRLs) would have its own utility and significance which can be used for optimizing the diagnostic imaging procedure.Radiation dose measurements were carried out by using high sensitivity,nearly tissue equivalent LiF:Mg,Cu,P thermoluminescent dosimeter chips applying the method of direct measurement during the irradiation of the patient.Adult patients aged over 18 years were only included in the study.The x-ray machine used in this study contains a high frequency x-ray generator and the image recording system was computed radiography system.The measurements were carried out on 125 patients (140 projections) and ESD values were estimated for each projection.The 3rd quartile value of the distribution of measured patient dose for a given projection was calculated to establish local/ institutional DRLs.A large variation was found in the measured ESD values for a given projection.The ratio of maximum and minimum ESD is found in the range of 2-11.The majority of the measured dose data were found smaller than the recommended national and international DRL values.The present study presents a new concept of establishing the local DRL the implementation of which is expected to reduce the patient dose and hence the population dose from medical diagnostic procedures.
