Numerical Simulation of Functional Evaluation for Implant-supported Complete Maxillary Denture with an Efficient CT Image Segmentation and Mesh Creation Technique

Objective: This study aims to create twelve numericalmodels of implant-supported attachment-retained completemaxillary dentures by an efficient CT image segmentation andmesh creation technique, as well as evaluate mechanicalcharacteristics of Model 1. Methods: Masks were first createdafter identifying regions of interest on CT scan images ofmaxillary prosthesis, then integrated with CAD models ofimplant, and finally meshed by extended volumetric marchingcubes (EVoMaC) approach. Von Mises stress of edentulousmaxilla restored with six anterior implants and resilientattachments placed distal to canines (Model 1) were determinedunder oblique, vertical and horizontal forces, respectively.Results: Twelve complete maxillary denture meshed models weresuccessfully developed, which could be imported directly into thefinite element package and analyzed without any manualcorrection. As to Model 1, maximum von Mises stress values ofperi-implant bone were observed on the mesial cervical areaaround loaded side canine implant under different loads.Furthermore, there were no significant differences betweenmaximum stress values of loaded side canine implant and those ofcentral incisor implant, both of which were more than those oflateral incisor implant under all loads. Conclusion: Thepioneering image segmentation and mesh development approachused in current study is an effective and successful technique innumerical simulation of functional evaluation for dentalprosthesis design optimization, and can be applicable tovisualization and simulation of other human bones andprostheses with complex internal architectures. In addition,simulated results imply that bone quality before healing and boneloss after treatment in canine region are of particular importanceto the long-term success of Model 1.
