INCIDENT INVESTIGATION AND ANALYSIS
Incident investigation and analysis is one strong circle in HSE management chain. A credible investigation and analysis is the key to take proper actions and improve the HSE performance. However, incident investigation and analysis is a weak point in many fields, people pay more attention to the event and immediate causes other than the root causes, and they focus on the consequence and blaming at persons who was involved in the incident, other than the investigation and analysis, and following up actions and recommendations implementation. The author, with his decade experience in oil field and his site people injured case, tell why incident investigation is not easy job and described the procedures and methodologies of incident investigation and analysis.
