Organizational Unsafe Behavior Control in Blowout Incident Prevention

This paper presents an incident cause analysis ofLuol6H H2S blowout in China. After identifying the main unsafe behaviors at each stage from the overflow to wild blowout, it shows that the direct cause of the blowout is the employeesviolation of work procedures and standards that can be defined as individual unsafe behavior, and the occurrence of individual unsafe behavior is due to the deficient management of organization that can be regard as organizational unsafe behavior, of which the latter is the root cause to this incident. The relationship of individual behavior and organizational behavior is re-verified by the statistic analysis of the 59 serious drilling blowout cases from 1970 in China. Finally, the paper discusses the feasibility of blowout prevention through the behavior-based safety approach. It is demonstrated that individual safety behavior is influenced definitively by organizational safety behavior, and organizational safety behavior is influenced profoundly by organizational safety culture. Therefore, in order to effectively prevent blowout incident, drilling companies should control organization unsafe behavior base on organization safety culture.
