Patient Safety Approach: Human Error Evaluation in a Hospital Pharmacy
Currently, patient safety has been one of key issue of healthcare industries in all over the world. The reliability of prescription preparing is one of topics about patient safety. The job of pharmacist is one of key jobs in healthcare industries. This object of this study was to analysis the job of pharmacist in hospital and to evaluate the rate of human error to promote patient safety in the hospital. Some designed questionnaires and the job analysis methods (Hierarchical Task Analysis (HTA), and human error analysis method (Systematic Human Error Reduction & Prediction Approach (SHERPA), self aware error types analysis method) have been used to collect the basic data of pharmacists and self aware of error to find the potential risk factor of human errors. The results show the figures of human HTA and SHERPA. The type of commission error was a major type of error and omission error was few. Most pharmacists thought commission and rule were major type of errors. Pharmacists have high education background and very familiar their jobs, but errors happened during unusual task or careless condition. The two key factors of human error were over occupational loading and lost attention. To increase human reliability of pharmacist, good job rotation, task scheduling of pharmacy department in hospital, and reinforce basic concepts of patient safety and prescription procedure were recommended.
