Results from the AHRQ Hospital Survey on Patient Safety Culture

The U.S. Institute of Medicine (IOM) identified changing the culture of healthcare organizations as one of the biggest challenges to moving toward a safer health care system. In 2004, the Agency for Healthcare Research and Quality (AHRQ) released the Hospital Survey on Patient Safety Culture (HSOPS), a hospital staff survey designed to help hospitals assess the culture of patient safety in their institutions. Since then, hundreds of hospitals across the U.S. and internationally have implemented the survey. The HSOPS Comparative Database is published on an annual basis providing information that enables hospitals to compare their data to facilitate internal assessment and learning in the patient safety improvement process and to compare against other hospitals. The HSOPS Comparative Database Report includes data from 622 hospitals trending results over time based on data from 204 hospitals that administered the survey multiple times. Hospitals that have administered the AHRQ patient safety culture survey have found it to be useful for internal feedback about their organizations patient safety culture and find the Comparative Data Three annual comparative databases have been published in 2007, 2008, and in 2009. The survey has been translated into 16 languages and used in 24 countries, and AHRQ will be developing an international comparative database on the survey.
