Safety management at the sharp end: goals conflict and risky trade-offs by radiographers in radiotherapy
The last barrier of defense in radiotherapy, before the actual treatment, is ensured by radiographers in the deliverance control room. The radiographers task goals are double:i) producing patients health, i.e. ensuring the deliverance of the daily care, avoiding cancellations of irradiation sessions and ii) producing patient safety, i.e. making sure that the care deliverance is done under the prescribed conditions, avoiding incidents/accidents. A conflict between these two goals appears in case of not-nominal situations. The objective of this paper is to highlight the trade-offs in risk management carried out by the radiographers through the analysis of 55 not-nominal situations management. In 47 of the 55 not-nominal situations, radiographers manage the conflict of goals in order to avoid having to make “sacrifice decisions aiming at supporting one of two objectives. In the 8 remaining situations, the management of the conflict was accomplished by a trade-off between the goals of patients health and safety. When making sacrifice decisions, radiographers take into account, on one hand, the risks and benefits for the patients, and, on the other hand, the treatment phase, knowledge about co-workers behaviour, and patients satisfaction.
