Analysis of Blowout Accident in Well Operation for Safety Management

Purpose, one of the most important activities of safety management is to maintain the safe work procedure in the workplace. Safety of work procedure is a function of many factors which shape human safety behaviors, and human safety behavior affect the safety of work systems simultaneously. For this motive, measuring performance shaping factors (PSFs)to enforce work system safety needs an overall approach. Method, as a case study, a blowout accident is analyzed through m-SHELL Model. Furthermore, the criticality of PIFs is determined by equation of structure criticality of fault tree analysis (FTA)within m-SHELL contexts. As a result, we found that performance planning before a Well operation, supervising to different work units, communication inside and among groups and characterized safety training on different working process have the main influences on safety of work system in a Well operation. This study suggests a list of its criticality measurement for safety management of work systems, when we functionally analyze and evaluate human errors to prevent blowout accidents with the perspective of social-technical science.
