A framework for human error analysis based on behavior modes & cognitive model

System safety could be degraded by human errors seriously. A framework is presented based on behavior modes & cognitive modes for human error analysis and reduction. Firstly, a classifier for behavior modes partition is presented. The specific mode can be determined guided by this classifier. Secondly, the relationships between the three behavior modes and cognitive modes are analyzed. The cognitive processes under the three modes are discussed respectively. Thirdly, the human error modes and their causes under various behavior modes are analyzed. And the measures for error reduction are discussed. Finally, the Steam Generator Tube Rupture (SGTR) accident scenario is selected as the example to illustrate the guidance capacity of the presented framework.
