Efficacy and safety of non-invasive ventilation in the management of chronic airway diseases

Non-invasive ventilation (NIV) refers to the delivery of assisted mechanical ventilation to the lungs without endo/orotracheal airways. As the techniques and devices of NIV develop, it has revolutionized the management of patients with various forms of respiratory failure. It has been widely used as short-term treatment for patients with acute respiratory decompensation, and long-term domicile rehabilitation therapy for those with chronic respiratory diseases. There have been an increasing number of large studies supporting the administration of NIV in patients with exacerbations of chronic obstructive pulmonary disease (COPD) and acute severe asthma. It has also been found to be effective for the weaning from invasive mechanical ventilation. Although it is an extremely useful technique, NIV cannot be adopted in all patients and correct patient selection is important. This review will focus on the recent literature and evidences pertaining to the use of NIV in chronic airway diseases including COPD and asthma, as well as the typical indications and benefits in the application of NIV.
