High number endometrial polyps is a strong predictor for recurrence-findings from a prospective cohort study in reproductive-age women
Objective: To compare the incidence of recurrence between high number endometrial polyps (EP) cohort (i.e.,>6) and single EP cohort among reproductive-age patients after polypectomy.Design: Prospective observational cohort study.Setting: Single university center.Patient(s): Premenopausal women who underwent hysteroscopic endometrial polypectomy were recruited.Patients with over 6 polyps found during the surgery were enrolled as the high number EPs cohort while those with single polyp were recruited as the single EP cohort.Intervention(s): Patients were followed up by transvaginal ultrasound scan every 3 months after polypectomy to detect EP recurrence.Main Outcome Measure(s): The study enrolled 101 cases of high number EPs and 81 cases of single EP.All baseline parameters were comparable except that the high number EPs cohort was slightly younger than that of single EP cohort (33.5”30.0-39.0”versus 36.0”30.5-43.0”,p<0.05).The risk of recurrence in high number EPs cohort was 4.08 (95%CI,1.89-8.81,p<0.01) times higher than in single EP cohort one year after polypectomy,showing 45.5% recurrence rate versus 13.4% respectively.High number EP (hazard ratio”HR”3.5;95%CI,1.4-8.5,p<0.01),endometriosis (HR2.4;95%CI,1.1-5.4,p<0.05) and past polypectomy history (HR2.2;95%CI,1.0-4.6,p<0.05) were associated with polyp recurrence independently.Conclusion(s): The high number EPs cohort was much more prone to recurrence than single EP cohort.High number EPs,endometriosis and past polypectomy history were independent risk factors for recurrence.High number EPs is suggested to be a distinct subgroup attributing to different pathogenesis which needs increased monitoring and prevention.
