Meta-analysis of functional magnetic resonance imaging resting-state brain functional connectivity in chronic pain
Objective:The objective of this meta-analysis was to describe functional magnetic resonance imaging(fMRI)changes of resting-state brain functional connectivity in chronic pain.Methods:A database search was performed in PubMed,Excerpta Medica Database(EMBASE),Web Science,Science Direct,Springerlink,and BrainMap for literature published from database establishment to June 2016.Based on inclusion and exclusion criteria,studies supporting fMRI analysis of the resting-state brain in chronic pain were collected.Any coordinates analyzed using the MNI system were converted to Talairach space using the Icbm2tal software. Finally,an ALE map was created using the GingerALE 2.3 software.Results:Twenty studies with a total of 389 subjects were identified from the literature.Regions of interest(ROIs)included the insula,cingulate gyrus,precuneus,inferior parietal lobule,periaqueductal gray matter,middle frontal gyrus,and paracentral lobule.Resting-state brain functional connectivity increased in the insula,cingulate gyrus,posterior central gyrus,paracentral lobule,middle frontal gyrus,and middle temporal gyrus,and it decreased in the paracentral lobule,precuneus,parahippocampal gyrus,anterior cingulate,superior frontal gyrus,middle frontal gyrus,inferior frontal gyrus,cingulate gyrus,inferior parietal lobule,medial frontal gyrus,and precentral gyrus.Conclusion: Our results suggest that resting-state brain functional connectivity is increased in the insula and related brain areas but decreased in the precuneus and inferior parietal lobule.Areas of both increase and decrease in activity included the default mode network,frontal lobe,and paracentral lobule.Therefore,there is a probable correlation between chronic pain and cognitive function.
