Clara Cell Protein 16 (CC16) levels are reflective of different pulmonary injuries, and it hasbeen indicated that exposure to occupational dust may lead to a reduction in CC16 levels. However, therehave been few studies focused on silica dust exposure. We aimed to assess the effect of occupational silicadust exposure on serum CC16 levels; we also determined the CC16 levels at silicosis diagnosis in order todistinguish between patients with and without silicosis. In total, 239 male workers were divided into threegroups as follows: 75 silicosis patients, 75 workers with silica dust contact, and 89 healthy controls with noexposure to silica dust. The blood serum was collected from all subjects and CC16 levels were measuredusing enzyme-linked immunosorbent assays (ELISA). The CC16 level at silicosis diagnosis was evaluatedusing a receiver operating characteristic (ROC) curve. The mean士 standard deviation (SD) serum CC16levels of the silicosis, silica dust contact, and control groups were 13.95± 1.66, 16.87± 2.11, 18.14± 1.05ng/mL (P<O.O01). The CC16 ROC area for the diagnosis of silicosis was 0.919 (P<O.O01) with a cut-offvalue of 16.21 ng/mL. The sensitivity and specificity of CC16 were 81.10% and 92.00%, while the kappavalue was 0.731. We found that CC16 expression is down regulated in silicosis patients suggesting thatCC16 may be involved in the pathogenesis of silicosis. CC16 can be used as a blood serum biomarker foraccurate diagnosis of silicosis.
