Efficacy of Oxaliplatin-and 5-Fluorouracil-based Chemotherapy followed by Gastric Surgery in Advanced Gastric Adenocarcinoma:A Retrospective Study

  Background and Objectives To assess the efficacy and safety of oxaliplatin-and 5-fluorouracil(5-FU)-based preoperative chemotherapy regimensin advanced gastric adenocarcinoma and to identify the factors affecting remission and survival.Methods We retrospectively evaluated the following in 85 patients with advanced gastric adenocarcinoma who underwent surgical treatment after chemotherapy with oxaliplatin-and 5-FU: chemotherapy response,type of surgical resection,pretreatment clinical stage,TNM stage,tumor size,tumor differentiation status,and patient survival.Results Each patient received 2-6 cycles of preoperative chemotherapy; 5 patients(5.9%)achieved complete remission,42(49.4%)achieved partial remission,33(38.3%)had stable disease,and 5(5.9%)had progressive disease.The response rate was 55.3%.The 3-year survival rate of the 55 patients who underwent curative gastrectomy before 2014 was 61.8%,with a median overall survival of 32.4 months and a progression-free survival of 29.7 months.TNM stage(pretreatment stage,depth of tumor invasion,lymph node metastases,distant metastasis,and pathological stage),differentiation status,tumor size,and preoperative chemotherapy response were significantly associated with patient prognosis(P < 0.05).Conclusion TNM stage,tumor size,and tumor differentiation were associated with preoperative chemotherapy response and prognosis.Preoperative chemotherapy with surgery improves the prognosis of patients with gastric adenocarcinoma.
