Primary intimal sarcoma inthe aortic arch and the descending aorta:a case report and literature review

  Aortic sarcoma is a rare entity.In this study,we report a case of a 56-year-old man with complaints of pain and numbness of bilateral lower extremities.Chest contrast-enhanced Computed Tomography(CT)and Computed Tomography Angiography(CTA)demonstrated severe stenosis in the aortic arch and the descending aorta.A histopathologic examination of the tissue was obtained through the endovascular biopsy and indicated an aortic sarcoma.Immunohistochemical analysis was positive only for vimentin.An endovascular aortic repair was performed to prevent the recurrent embolic events.However,the patient was readmitted for intermittent claudication and the thrombus in the stent in the 6th month after the first operation.Axillary bifemoral and femoro-femoral cross-over bypass surgeries were then performed to supply blood to the lower extremities.In conclusion,multiple imaging approaches should be taken to detect the primary lesion when artery embolic events or intermittent claudiation happen.Aortic sarcoma should be taken into consideration as one of the possible etiologies.Palliative surgeries such as bypass,endovascular aortic repair can also be used for improving quality of life.
