Endovascular repair of complex aortic pathologies with the sandwich technique in nine patients
Aim This study aimed to evaluate the safety and efficiency of the sandwich technique used in endovascular repair of complex aortic pathologies.Methods Nine patients(mean 72.7 years old,ranging from 60 to 85 years old)with complex aortic pathologies were studied retrospectively from January 2013 to September 2016 in two vascular centers of teaching hospitals.Computed tomography angiography(CTA)was performed to make individual therapy.They were performed endovascular repair with sandwich technique,including one with the sandwich,chimney and fenestrated technique at the same period.All patients were followed up at 1 month,3 months,6 months,12 months and yearly thereafter with ultrasound and/or CTA.Results The technical success was 88.9%.At the final angiography,a Type Ⅱ endoleakand two Type Ⅲ endoleaks were found.No significant changes were observed regarding to renal function(Preoperative eGFR:74.61±21.36mL/(min·1.73m2),postoperative eGFR:75.77±19.75 mL/(min·1.73 m2),p=0.826).One patientsuffered myocardial infarction and pulmonary infection postoperatively.During the follow-up(Mean 16 months,ranging from 2 to 36 months),a Type Ⅱ endoleakoccurred in one patient one week after the operation,whichshrank ten months later.One patient was readmitted for right brachial artery pseudoaneurysm three months after discharge.One suffered paresthesia.No paralysis,claudication or bowel ischemia happened.The mortality was 0%.Conclusion For patient who are not candidates for open surgery or conventional endovascular repair with complex pathologies,the sandwich technique is a feasible and safety method.
