Medication Reconciliation to Ensure Drug Safety in China:A Case Report

摘要：

　　Objective To discuss clinical pharmacist” role in transitions of medicine care by medication reconciliation.Case A case of multiple diseases patient by medication reconciliation in China.Specific objectives included the following: adjust medication in different stages,avoid repeat medication and potential medical risk to ensure drug safety.Results Clinical pharmacist cared a case of an elderly patient with medication reconciliation at admission,transfer and discharge,to dosing regimen optimization,application of essential drugs and discontinuation of non-essential drugs.Medication reconciliation can avoid repeated drug use,adjust drug variety,dosage and route of administration,enable the patient to obtain the necessary drug treatment in different stages of the disease,reduce medicine numbers and cost and potential drug risks.Conclusion Medication reconciliation has a positive effection with patient of multiple diseases especially for the elderly,clinical pharmacists in China can set up the suitable ways of medication reconciliation.
