Using the SOAP model in pharmacist consultation: Application of what I learned during 6 month clinical pharmacy training at Samford University in the US
Background In providing health care services,there is a chance for pharmacists to provide integrated,cost-effective pharmaceutical services to patients.These pharmacy services are patientfocused and involve rounding with the medical team to provide drug information,assist withmonitoring patients” responses to their medications,provide patient education in-hospital andat discharge,and make therapeutic recommendations in consultation with physicians.Themost challenging part of this process is how to make the appropriate clinical decision onindividual patient therapy and what comments should be included in the written consultation.To further my knowledge and skills in these areas,I spent six months in training as a visitingscholar at the McWhorter School of Pharmacy,Samford University,Birmingham,Alabama,USA.
