Study on the Effection of Clinical Pharmacist Through an Elderly Resident Patient by Medication Reconciliation

摘要：

　　OBJECTIVE to investigated the effection of rational use in patient of multiple diseases by medication reconciliation.METHODS Clinical pharmacist cared a case of elderly patients with medication reconciliation at admission,transfer and discharge,to dosing regimen optimization,application of essential drugs and discontinuation of non essential drugs.RESULTS medication reconciliation can avoid repeated drug use,adjust drug variety,dosage and route of administration,enable the patient to obtain the necessary drug treatment in different stages of the disease,reduce medicine numbers and cost and potential drug risks.CONCLUSION medication reconciliation has a positive effection with patient of multiple diseases especially the elderly,domestic clinical pharmacists can set up the suitable ways of medication reconciliation.
