Conservative tuina treatment of a qigong practitioner with inguinal canal pain: an atypical piriformis syndrome case study

  Objective: The purpose of this article is to discuss an atypical case of piriformis syndrome that was repeatedly misdiagnosed and finally treated successfully with tuina; discussion of atypical cases can help to avoid similar misdiagnoses in the future.Case Report: A 30-year-old male patient with a chief complaint of right inguinal canal pain,when the pain was severe it caused limping or complete inability to walk.The complaint began 18 months ago as a result of improper posture during qigong practice.The patient did not however exhibit low back pain,passive buttock pain,radiating pain,pain at any degree of elevation during the straight leg test,and was therefore misdiagnosed multiple times.Upon further evaluation,the patient was found to exhibit tenderness in the piriformis region when pressed and both of the piriformis tightness tests were positive in the afflicted leg.Intervention and Outcome: The patient was treated with tuina twice,primarily using circular kneading,plucking and assisted hip-rotation methods.The first treatment decreased the pain by a reported 60% and the second by 30%,leaving the patient able to walk and train freely without pain or limitation of range of movement.Conclusion: An atypical case of piriformis syndrome was successfully treated by tuina in two sessions,after having been misdiagnosed and erroneously treated multiple times.
