The Efficacy of Acupuncture Treatment for Adult Migraine Patients: A Meta-Analysis of Randomized Clinical Trials

”Objective” To comprehensively summarize the outcomes found in the included randomized controlled trials of acupuncture as a treatment option for adult migraine patients.”Method” This analysis was conducted by three reviewers who selected randomized clinical trials from the EBSCO Host electronic database at the University of the Incarnate Word Library. The database included PubMed, MEDLINE, CINAHL Complete, Cochrane Library CENTRAL, Alt.Health Watch, Academic Search Complete, Psych INFO, Health Source: Nursing/Edition,Psychology and Behavioral Sciences Collection, and SPORT Discus Online Journal. The search duration started in February 2014, and ended in May 2015.Randomized controlled trials (RCTs)on acupuncture treatment for migraine attacks and prophylaxis were assessed by their methodological quality and relevant data.Risk of bias was determined by the Cochrane risk of bias assessment tool.Eligible RCTs compared acupuncture with sham acupuncture, drug treatment, or no-acupuncture.Primary outcomes included a responder rate (reduction of at least>50% in headache days with moderate or severe intensity, number of migraine days, or number of migraine episodes), number of migraine days, 24 hour pain freedom, and analgesic use.Review Manager Software was used for the statistical analysis which included risk ratios (RR), mean difference (MD), or standard mean difference (SMD) with confidence intervals (CI) set at 95%.”Results” Eighteen randomized controlled trials (N=3,924 participants) were included in the data analysis. When Acupuncture was compared to Sham Acupuncture, more patients in the acupuncture group reported a decrease in migraine days (P=0.03), and complete pain freedom in a24 hour period (P=0.0001).However, in the analysis of 50% Response Rate and Analgesic Use,there were no significant differences between the acupuncture and sham acupuncture group. In another comparison, the acupuncture group had significantly more responders than the drug group(P<0.00001), and also proved to be superior in decreasing migraine days (P<0.00001).There were also more responders in the acupuncture group compared to no acupuncture (P<0.00001).”Conclusion” Acupuncture can be efficacious in treating migraine patients. However given a high rate of statistical heterogeneity, one must interpret these findings at their own discretion.Nonetheless, we believe that this meta-analysis has rendered enough significant evidence to recommend acupuncture as an evidence-based treatment option for migraine patients.
