The value of treadmill test to evaluate asymptomatic very late in-stent restenosis on patients after PCI

Background At present, PCI has become one of the most effective means for coronary heart disease, since 1977,Gruentzig performed the first cases of PTCA, Restenosis has always affects the long-term prognosis of interventional therapy.The DES was began to use in clinical application in 2000, but the incidence of stent restenosis is still at about10%.It is estimated that it will operate PCI for millions every year all around the world, of which 80%-90% use DES.And it is different from BMS, the restenosis rate of DES is higher than BMS after one year, it was named ”late-catch up phenomenon” about two-thirds of the patients with diabetes or abnormal glucose tolerance, most of these people have no obvious chest pain, dyspnea and other clinical symptoms, that is to say: asymptomatic myocardial ischemia.So is not easy to detect the clinical evidence of in-stent restenosis.Coronary artery angiography is the gold standard for the diagnosis of coronary heart disease, especially in-stent restenosis, but it is an invasive method and have a certain risk.Higher cost, and exercise treadmill test (TET) can reflect the change of myocardial ischemia is clinically is simple,convenient and feasible and non-invasive methods for the diagnosis of coronary heart disease.But now the value of exercise treadmill testing for the diagnosis of very laterestenosis after stent implanted (more than one year) is not certain.
