Feasibility and safety of coronary angioplasty by means of percutaneous transbrachial approach

Background Transradial coronary angioplasty has many advantages.But due to its high failure rate and restriction of incapable of satisfying large diameter tubes, the approach cannot be widely used.In recent years the transbradial approach has been increasingly employed as an alternative approach to percutaneous coronary intervention (PCI).The aim of this study was to assess the safety, feasibility and efficacy of transrachial coronary angioplasty.Methods We studied 356 patients who underwent PCI in 2008-2012.The transbrachial approach (group A) was used in 210 consecutive patients, who were failed to undergo transradial access.The transradial approach (group B)was used for vascular access in the remaining patients (n=146).The same puncture process and material was employed.Results The brachial access was achieved in all patients of group A.The cannulation time (from skin anesthesia to the time of arterial cannulation), total procedure time (from patient arrival at the catheterization room to the completion of the procedure), amount of contra ST-agent, stent amount, vascular and systemic complications, and short-term effect did not significantly differ between group A and group B.Conclusions The transbrachial approach can represent a safe and feasible method for performing angioplasty with similar results of transradial approach and it can be widely applied.
