Impact of temporary right ventricular pacing on right ventricular function in patient with pulmonary arterial hypertention
Background In the modem management era, pulmonary arterial hypertension remains a progressive, fatal disease, right heart failure is an important cause of morbidity and mortality in PAH.Current PAH medication (prostacyclines, endothelin receptor blockers, phosphodiesterase (PDE)-5 inhibitors and calcium antagonists) focuses on controlling the excessive vascular remodelling, resulting in a reduction in right ventricular (RV) load.At present, no treatments are currently available that directly target the right ventricle.Cardiac resynchronization therapy (CRT) has been proved as an effective treatment for patients with moderate to severe systolic heart failure and left ventricular dyssynchrony, some studies demonstrated the patients with pulmonary arterial hypertension have diastolic interventricular delay (DIVD) and right ventricular activation delay, so electrical resynchronization may be a new useful method for treatment of RV failure. This study aim to exam temporary pacing will can improve RV function in patient with PAH or not.
