Primary treatment of clefts, with a new approach to complete bilateral clefts
After 25 years of working with Ralph Millard, the originator of the rotation/advancement lip repair, the author will go over the cunrent approach for clefts of the lip and palate used at Miami Children”s Hospital, ranging from incomplete unilaterals, complete unilaterals, incomplete symmetric and asymmetric bilaterals, and complete bilaterals.The role of naso-alveolar moulding, which has supplanted the Latham/Millard apparatus, will be discussed.This permits the closure of the alveolus by gingivoperiosteoplasty, and anterior palate witha vomer flap at 6 months of age, with the remainder of the palate being closed at 18 months.
