Evaluation of the Pathological Grade in Gliomas:Application of DSC-PWI

  objective Assessment of DSC-PWI For gliomas preoperative value of histological grade.Materials and Methods 76 cases gliomas confirmed by surgery and pathology were analyzed,36 cases of low-grade(grade Ⅰ~Ⅱ),40 cases of high-grade(grade Ⅲ~Ⅳ),all cases underwent conventional MRI plane scan,contrast MRI scan and DSC-PWI.Maximum rCBV values of solid part(the solid part to the corresponding contralateral normal white matter ratio)are caculated.Comparison of maximum rCBV values of tumor different levels was assessed by t test.Drawing ROC curve to determine the ideal value of rCBV threshold between high-grade and low-grade gliomas.Results The maximum value of rCBV in the solid part of gliomas are related positively with pathological levels,the maximum rCBV values of high-grade and low-grade gliomas were 0.87411±0.935、2.0415±1.5311 respectively,significant difference were showed among all grades of gliomas(P=0.002).To identify the high level and low-grade gliomas,1.35 as the ideal threshold,the AUC value 0.73911.the sensitivity and specificity were 66.7% ,73.0% .Conclusion rCBV examination will be able to assess the level of tumor pathology.PWI examination for preoperative evaluation of gliomas have a higher value of histological grade.
