部分切开留皮桥术式治疗弯曲型肛瘘的临床研究
Objective To observe and appraise the curative effect of Therapy for bending-type Anal Fistula by Partial Incisin and skin-Retention Technique.Methods Sixty patients with bending-type Anal Fistula were picked out and divided into the treatment group (30 cases) and the control group(30 cases) using a random number table.The treatment group use Partial incisin and skin-Retention therapy,while the other use anal fistulotomy.Observe the difference of wound area between two groups after the surgery and the 2nd,5th,9th,14th day after the surgery and the Maximum width of the wound after the surgery;observe and made records of the point of anal aches, wound exudate in 6h,the 1th,2nd,3rd and 7th day after operation;made records of the patient with anal leakage after operation and the time when the wound healed;Made records of wound healing time,scar size and anal deformation.Results 5 patients was lost in the end of the research,The total effective rate of the two groups was both 100%.The difference of wound area between two groups after the surgery and the 2nd,5th,9th,14th day after the surgery is significant (P=0.000,P<0.05);The average width of the wound of A group is 2.41 ± 0.20cm,while B group is 3.01±0.23cm,the difference is remarkable (z=-6.117,P=0.000,P<0.01);The average healing time of A group,is 25.26 ± 1.51d,and the B group is 30.32 ± 1.92 days, with a significant difference (P=0.000,P<0.05);The mean scar area of A group was 6.68 ± 1.62cm2, and B group is 7.96 ± 1.95cm2, with a significant difference (P=0.011,P<0.05);1 Patient has anal deformation in A group while 8 patients in B group,the difference is remarkable (P<0.05).1 patient with anal leakage was found in A group and 9 in B group,the difference is remarkable (x2=4.945,P=0.026,P<0.05).But there is no remaikable difference in the terms of anal aches and wound exudate (P>0.05).Conclusions A group is better than B group in reducing the wound area,Maximum width of the wound,scar size, healing time,anal sphincter and ana deformation.Compared with anal fistulotomy, The partial Incisin and skin-Retention Technique have same advantages in diminishing the wound area, scar size, healing time, pretecting anal function.
