Management of Dependent Failures
Dependent failures are typical causes of severe accidents.Section 2 describes four types of dependencies:1)functional,2) human-interaction,3) spatial,and 4) component hardware failure.These types are not mutually exclusive but four overlapping facets of dependent failures,inspiring human creative thinking for safety.Section 3 focuses on failures caused by similarities,i.e.,common-cause failures.Typical similarities are exemplified as coupling mechanisms.Section 4 gives provisions for each type of dependenies.The Fukushima nuclear power plants accident is a manifestation of dependent failures caused by a mechanism called spatial dependency that should have been corrected from a PRA (probabilistic risk assessment) pointed of view.
