Treatment of osteonecrosis of the femoral head by using greater trochanteric bone flap pedicled with double blood vessels

  Vascularized bone grafting has been reported to be successful for patients in early-to-medium stage of Osteonecrosis of the femoral head (ONFH).We examined the vessel morphology and blood supply of the greater chanter area and evaluated the possibility and feasibility of reconstruction of the femoral head by using bone flap pedicled with tansverse and gluteus medius branches of lateral circumflex femoral artery.Based on the anatomy study,from Jan 2002 to May 2004,thirty-two ONFH patients treated with bone flap pedicled with double blood vessels were followed up completely.Fifteen hips were in Ficat and Arlet stage Ⅱ and 17 were in stage Ⅲ.The mean follow-up was 99.5 months.Two of the 32 patients required a total hip replacement due to severe hip pain.The overall HHS improved from a mean of 55.2 points to 85 points.Our data suggest the technique is relatively simple,requires no special surgical technique,and eliminates donor-site morbidity and is useful for young patients of Stages Ⅱ to Ⅲ disease without or with mild collapse of the femoral head.
