Balloon kyphoplasty versus percutaneous vertebroplasty in treating osteoporotic vertebral compression fracture:a meta-analysis

OBJECT:To assess the safety and efficacy of balloon kyphoplasty(BP)compared with percutaneous vertebroplasty(VP),in the treatment of osteoporotic vertebral compression fractures(OVCF).METHODS:A systematic search of studies published between 2000 and Mar 2012was conducted using MEDLINE,EMBASE,OVID,ScienceDirect and Cochrane Review databases.Randomized controlled trials(RCTs)and prospective comparative data on safety and clinical effects were identified.Two study authors independently reviewed the 231 articles originally identified and selected 10 for analysis.Study title,demographic characteristics,numbers of vertebral body,adverse events and outcomes were extracted manually from all selected studies.RevMan 5.1 software was used for meta-analysis.RESULTS:10 studies encompassing 783 patients met the inclusion criteria.Overall,the result of meta.analysis indicated that there were significant differences between two groups in long-term kyphosis angle(WMD=-2.64,95%CI-4.66,-0.61;p=0.01),anterior height of vertebral body(WMD=3.67,95%CI 1.40,5.94;p=0.002),cement leakage rates(RR=0.70,95%CI 0.52,0.95;p=0.02).However,there were no sigIlificant differences in short-term visual analogue scale WAS)scores(WMD=-0.57,95%CI.1.33,0.20;p=0.15),long-term VAS scores(WMD=-0.99,95%CI-2.29,0.31;p=0.14),short.term Oswestry Disability index(ODI)scores(WMD=-6.54,95%CI-14.57,1.48;p=0.11),long-term ODI scores(WMD=-2.01,95%CI-11.75,7.73;p=0.69).operation time(WMD=4.47,95%CI-0.22,9.17;p=0.06),short-term kyphosis angle(WMD=-2.25,95%CI-5.14,0.65;p=0.13),adjacent-level fracture rates(RR=1.52,95%CI 0.76,3.03;p=0.24).CONCLUSIONS:The results of this meta-analysis demonstrate that KP is a safe and effective surgical procedure in treating OVCF.Compared with vp,KP can relieve long-term kyphosis angle,improve height of vertebral body,and reduce the incidence of cement leakage,while it cannot improve pain and function scores,and decrease adjacent-level fracture rates.However,Because of the limitations of this meta.analysis.a large randomised controlled trialiS required.
