Thoraco-abdominal aorta revascularization through a retroperitoneal approach: 8 cases report in Macau
BACKGROUND: The transperitoneal approach to the aorta is the most widely accepted surgical approach in aortic surgery as it is simple, fast and provides excellent exposure of the intra-abdominal cavity and vascular structures. In recent years, there has been an increasing interest in the retroperitoneal approach to the aorta. METHODS: A retrospective report to evaluate this surgical approach.Data of 8 consecutive patients in Macau who presented with aortic diseases from 2007 to 2008 treated with the aorta repair were analyzed. Demographic features, intra-operative and postoperative data were analysed and compared according to the approach used. 1 cases of thoracoabdominal dissection and 4 cases of abdominal aneurysm were received artificial graft replacement. Hybrid iliac artery restitution with Zenith stent graft deployment covering the ostium of the LSA was performed in 2 cases of infra-renal abdominal aneurysm. Aortic iliac artery bypass was performed in 2 cases of aortoiliac occlusion.RESULTS: There was no operative or early postoperative death. No perioperative intestinal adhesion, or ureteral obstruction was found. One case of postoperative complication was noted for graft infection. The finnally recovered 4 months later. CONCLUSIONS: Retroperitoneal approach is a safe and feasible technique, which associated with a significantly lower incidence of postoperative pulmonary complications, rapid recovery of gastro-intestinal functions, shorter ICU and hospital stay, less peri-operative blood loss and lower means effort-pain scores.
