Eztended surgical resection plus percutaneous isolated hepatic perfusion for multiple hepatocellular carcinoma with portal vein tumor thrombus
Complete removal of the tumor by either hepatectomyor liver transplantation currently represents theonly formof potentially curative treatment for hepatocellularcarcinoma(HCC)．However，only 10 to 30％of patientspresentwith resectable tumors，and systemic chemotherapy ismarginally effective because the narrow therapeuticratio preeludes dose escalation．This situation has led us todevelop a novel alternative method of drug deliverywith percuta-neous isolated hepatic perfusion(PIHP)．The techniqueavoids extensive surgical mobilization and dissectionof theliver to achieve a dose-intensified intraarterial chemotherapy．
