Protective Effect of intracisternal Lidocaine injection During SubaraChnoid Hemorrhage—Induced Cerebral Vasospasm

Many literature indicate the neuro-protective effect of fidocmne in models of ischemic brain injury.We hypothesized that administration of lidocaine after the onset of subarachnoid hemorrhage(SAH)would attenuate cerebral vasospasm and hippocampal CA1 pyramidal neurons impair in a rabbit model of subarachnoid hemorrhage.Methods:Thirty New Zealand white rabbits were randomly allocated to sham group(sham),subarachnoid hemorrhage group(SAH),and Lidocaine therapy group(Lidocaine)(n=10,respectively).All animals underwent operation under general anesthesia.1.5ml autologous nonheparinized arterial blood was injected into cisterna magna in SAH group and Lidocaine group,and 1.5ml saline into cisterna magna in sham group respectively.Thirty minutes after SAH.0.1ml saline was injected into cisterna magna in sham group and SAH group,and 0.1 ml 2%Lidocaine into cisterna magna in Lidocaine group,respectively.72 hours after SAH,All animals were euthanatized and in situ perfusion/fixation.Their basilar arteries and hippocampus tissue were harvested for morphometric analysis.Measurement of endothelin(ET),calcitonin gene-related peptide(CGRP)and interleukin-6(IL-6)levels was measured by Radioimmunoassay and ELISA at pre-operation and 72 hours after SAH.Result:The baseline level of ET,CGRP,ET/CGRP,IL-6 show no significant difference in three groups (P>0.05).But the level of ET and IL-6 in SAH group was higher than others(P<0.05)at 72 hours after SAH.Compared to the baseline,the level of ET, IL-6,and ET/CGRP was significantly higher(P<0.05),and lower level of CGRP in every group(P<0.05)at 72 hours after SAH.The cross-section areas and diameter of basilar artery were smaller and the normal neuron density of hippocampus were lower in SAH group than that of the other two groups (P<0.05).The cells of expressing C-fos in hippocampus were more abundant in SAH group than others(P<0.05).Conclusions:The study indicates that intracistemal administration of Lidocaine Can prevent the cerebral vasospasm and hippocampus tissue damage after subarachnoid hemorrhage in rabbit.
