Diagnostic value of whole-body MRI with diffusion-weighted sequence for detection of peritoneal carcinomatosis in colorectal malignancy
Objective Purpose of our study was to assess the diagnostic accuracy of whole-body MRI with diffusion-weightedsequence(WB-DWI)to determine the peritoneal cancer index(PCI)in correlation with surgical and histopathological findings.Materials and methods 12 of 15 patients underwent preoperativeWB-MRI Followed by cytoreductive surgery for primary tumorsof the appendix(n = 4),colorectum(n = 8)and their peritoneal disease.A total of 156 regions were retrospectively reviewed by two experienced radiologists without the clinical information.The sensitivity,specificity,accuracy were calculated at 13 anatomic sites.The WB-DWIPCIand PCI type were compared with surgical and histopathological findings.Results The paired t test resulted in nostatistical difference between the WB-DWI PCI and surgicalPCI(P = 0.115).WB-DWIcorrectly predicted the PCI type in 11 of 12 patients with a high accuracy of 91.7%,including 6 of 6 patientswith small-volume tumor,4 of 5 patients with moderatevolume tumor,and 1 of 1patients with large-volume tumor.WB-DWIcorrectly depicted tumors in 65 of 79 regions with 14 false-negative regions and 18 false-positive regions.The overall sensitivity,specificity,and accuracy of WB-DWIfor the detection of peritonealtumors were,respectively,82.3%,74.7%and 79.5%.For lesions < 0.5 cm in diameter,WB-DWI shows a good sensitivity of 69.0%.Conclusions WB-DWI is a valuable workup tool to evaluate the PCI before cytoreductive surgery in patients with colorectal peritoneal maligancy.
