Medicare Part D provides outpatient prescription drug insurance to older and disabled adult beneficiaries.Medication Therapy Management (MTM) is a mandated component of the Medicare Part D drug benefit program implemented to help patients with multiple chronic conditions, high drug costs, and high utilization improve the effectiveness and safety of their medication treatment.In 2006, the US Center for Medicare & Medicaid Services incorporated a MTM Program (MTMP) requirement for individuals with Part D coverage to ensure that drug regimens provide optimal therapeutic outcomes.
